Acknowledgment and Assumption of Risk

I, the undersigned, acknowledge that participation in any sports, physical training, or athletic activities during an Chapman Basketball Event involves inherent risks, including but not limited to, physical injuries (including serious injury or death), accidents, collisions, equipment failure, environmental conditions, or the actions of other participants or staff.

I voluntarily assume all such risks, both known and unknown, even if arising from the negligence of Chapman Basketball Academy or others, and assume full responsibility for my participation (or the participation of my child/ward, if applicable).

Waiver and Release of Liability

In consideration of being allowed to participate in activities, programs, and use of facilities provided by Chapman Basketball Academy, I hereby agree to the following:

1. I waive, release, and discharge Chapman Basketball Academy, its owners, directors, officers, employees, coaches, staff, volunteers, agents, and representatives from any and all claims, liabilities, demands, causes of action, or damages arising from personal injury, illness, death, or property damage resulting from participation in any way connected to Chapman Basketball Academy.

2. This release applies to any and all losses, damages, or injuries that may be sustained while participating in training, competitions, events, travel, or while on Chapman Basketball premises, whether caused by negligence or otherwise.

3. I agree not to sue or bring any claims against Chapman Basketball Academy or its representatives for any personal harm or injury sustained during participation.

Medical Treatment Authorization

In the event of an emergency, I authorize Chapman Basketball Academy personnel to secure from any licensed hospital, physician, and/or medical personnel any treatment deemed necessary for the participant’s immediate care and agree that I will be responsible for the payment of any and all medical services rendered.
